The Patient Safety and Quality Improvement Act of 2005 was incorporated into law in response to growing patient safety concerns in the United States. The aim of the law was to improve patient safety by encouraging hospitals and their staff to voluntarily report events that adversely affected patients. The Joint Commission on the Accreditation of Healthcare Organizations is a non-profit organization that accredits hospitals that meet the standards of the National Patient Safety Goals of the Joint Commission.

Joint Commission Objective 2 states that "ineffective communication is the most commonly cited root cause of sentinel events" and requires hospitals to "implement a standardized approach to handover of communications, including the ability to ask and answer questions" . Software has been developed to help hospitals obtain accreditation from the Joint Commission while protecting hospitals and other medical organizations from communication errors that could result in patient injury or death, as well as lawsuits against the caregiver.
